Steps for Veterans Requesting to Be Seen by a CCN Provider

#1 Contact Your Primary Care Provider
The preferred and most efficient way is through the VA’s healthcare online messaging system. Let your provider know you would like them to submit a referral for an Audiology appointment.
#2 Confirm the Referral
Once your provider confirms the referral has been submitted, please allow 2–3 business days for the Community Care Network (CCN) to receive it.
You may contact the CCN at 210-949-3850 (select option 1) to verify receipt of your referral.
#3 Request Your Appointment
When speaking with the CCN, request to schedule your audiology appointment specifically with:
